Making Heart Failure Treatment Simpler: A Landmark Cardiovascular Study from Sri Lanka
A landmark multicentre randomized clinical trial from Sri Lanka, evaluating a polypill implementation strategy for heart failure with reduced ejection fraction (HFrEF), has been published in JAMA Cardiology. The study is being conducted across 14 tertiary care hospitals, with a planned enrollment of more than 1,400 participants, and is being carried out in collaboration with the Washington University School of Medicine in St. Louis.
Why a polypill?
Patients with HFrEF often require multiple guideline-directed medical therapies, including complementary drug classes that have been shown to reduce mortality and heart failure hospitalizations. Yet, translating these evidence-based therapies into routine clinical practice remains challenging.
Multiple medications can mean greater pill burden, complex prescribing and titration, monitoring requirements, cost and access challenges, and difficulties with adherence.
The polypill approach combines multiple cardiovascular therapies into a single medication strategy, with the goal of simplifying treatment and facilitating the delivery of guideline-directed therapy.
Why this study matters
Its importance extends beyond the polypill itself. By evaluating an implementation strategy within real-world clinical settings, the study seeks to generate evidence that is relevant to healthcare systems facing the practical challenges of delivering complex cardiovascular treatment at scale. This is particularly important for low- and middle-income countries, where access, affordability, medication burden and healthcare-system capacity can influence how effectively guideline-directed therapies reach patients.
